Sending Over Your Patient Data

As soon as you’ve submitted your Setup Form, we’re ready to receive your patient demographic data.

We load your patient demographic data into the ChartCapture Cloud to ensure that it’s there when you’re ready to scan.

Want to do it yourself? You can upload your patients quickly and securely with our Import Wizard.

If you’re planning on purchasing, or if you have already purchased an EMR or EHR system, ensure that the Patient ID matches in both systems. If you’re not sure which Patient ID you should use, or how to get your patient data extracted, a quick email to support@chartcapture.com will answer any questions or concerns.

Note: If you’re able to provide demographics for ONLY the patients you plan to scan, searching for patients will work faster! The more you have to search through the longer it takes.

How do I get my Patient List?
If you’re not able to export the list yourself, let us know how to get in touch with your account manager and we’ll handle it for you.

Do not send your patient list via email. Send it to us securely using our File Share feature. Demographics should be delivered in an Excel spreadsheet or in CSV format.

These are the fields that we require:

  • Last Name
  • First Name
  • Middle Initial
  • DOB (mm/dd/yyyy)
  • Patient ID

What if get more patients?

If you add new patients to your EHR/EMR, they will not automatically update in ChartCapture. There are two ways to make sure you’ve got the same patients in both systems:

  1. If you’ve added very few patients, you can manually add them to ChartCapture.
  2. If you’ve added very many patients let us know and we’ll request an updated list from your EHR/EMR.
